With the sudden modernization of systems, technology becomes a mandatory tool for companies and organizations, whether private or not, that optimize processes and activities. Due to this new requirement and new technological advances, the tendency for these companies or organizations is to increasingly resort to computerized software, which offer greater agility and cost reduction for such. With this proposal in mind, the project presented aims to develop a prototype of a web application that manages data for the registration of patients at the institution CT Cruz Azul in Panambi, which welcomes drug and alcohol dependents, with the purpose of reducing costs with physical materials and automating the entire patient registration process. The application prototype will be developed to use responsively, using hypertext processor technologies such as PHP and Visual Studio Code as the development platform.
